If you are a first time customer to one of our locations a couple things to note.
- We bill primarily to your insurance, in some instances cash pay is an option. This makes us well versed in insurance requirements to maximize your coverage.
- Be sure to have a copy of the following forms on hand as they are needed in order to bill your insurance!
- Medical Insurance Card
- Detailed prescription from your physician detailing the services and/or equipment needed.
- Standard Written Order (SWO) – A standard form with detailed information
- Medical Records – Specific information that is needed in a patient’s medical records
- Don't have those on hand? Don't worry we will work with your physician's office to get the correct forms!
